Partners in Care collaborates with providers to build interventions that merge social supports with healthcare delivery. In this interview, founder June Simmons shares how Partners in Care designs tools for chronic disease self management and preventative lifestyle choices that keep people healthier and out of the cycle of emergency room visits and hospitalizations. Partners in Care also designed technology enabled medication reviews and home assessments that can be conducted by an alternative workforce to leverage a skill task alignment that makes the evidence based programs efficient and cost effective.
